Release and Waiver Statement
Group Name: Indianapolis Chinese Performing Arts, Inc. (ICPAI) Activities: Practice, show and dance, exhibitions, and all group-organized activities. Location: Public facilities, community facilities, individual’s properties, and any other locations for group activities including transportation vehicles. Time: Any time for group activities. I voluntarily join the ICPAI activities. I understand that any activity involves potential hazard and injury risks and ICPAI is not responsible for any of the risks. I assume that all risks associated with my voluntary participation in this group, including, but not limited to, falls, twists, traffic accidents and the effects from natural hazards in all the locations, all such risks being known and appreciated by me. Knowing these facts, I for myself, my heirs, or anyone else who might claim on my behalf, covenant not to sue and waive, release and discharge ICPAI and it’s representatives or affiliations for any and all claims or liability, whether foreseen or unforeseen, for death, personal injury, or property damage arising out of, or in the course of my participation in all the group events and activities.
